Patient forms are available for your convenience here on our web site. You can print the forms to fill out and bring into our office with you, or if you like, you may do the patient forms online.

Office Hours

Our Services

We provide comprehensive eye care service for your entire family. We offer glaucoma and cataract testing, a thorough analysis and treatment for eye diseases, vision training for children, low vision care for patients with deficient vision, foreign body removal and emergency service is always available.

We specialize in all types of contact lenses - lenses correcting astigmatism, disposable daily wear, extended wear and bifocal.

We also offer eye wear for specific occupations, such as pilots and those who work with computer terminals.

Dr. Powell & Dr. Gildersleeve are primary health care providers who diagnose, manage and treat conditions and diseases of the human eye and visual system.
